Which Form Should I Download?

To download the form to your computer, right-click on the form and select Save Target As (Macs use Ctrl + Click). Select the location where you want to save the downloaded form (such as the Desktop), and click on Save. When the download is complete, click on Open if you want to open the form and begin using it, or click on Close if you want to work on the form at a later time.

Microsoft Word
If you use Microsoft Word, download the Word format (doc). You will be able to type your information right on the form. Use the TAB key or the mouse to move from field to field. The document is locked so you cannot rearrange the form in any way. You are limited to a certain number of characters in each field.

Acrobat Reader
If you prefer to fill out the form by hand using a typewriter (Note: all application forms must be typed), you can download the Acrobat Reader format (pdf). You, of course, have to have Acrobat Reader on your computer to open the form. Acrobat Reader is available for FREE from this website:

Rich Text format
If you use another word processing other than Microsoft Word, such as Word Perfect, or ClarisWorks or Appleworks (Mac), you can download the Rich Text Format (rtf). (Note: WordPad will also open RTF files. WordPad is located in the Accessories folder - Go to Start... Programs... Accessories). If you use this option, please note that the form is not locked and you must be careful not to rearrange the line spacing. Also, you may need to insert or delete lines so that the form prints correctly. Do a Print Preview to see how the form will print and make necessary adjustments.
